Does insurance cover my physical therapy through Direct Access?

It depends on your insurance provider and coverage. We would be happy to verify your benefits for you free of charge. You are able to fill out the form to your left or call XXX-XXX-XXXX to speak to one of our patient representatives. 

Who is eligible to be seen via Direct Access?

Any person in XXX is eligible to be seen directly by a physical therapist. You do not need a prescription/referral from a physician. We can schedule you for an evaluation today to get you back on track. 

What insurances do you accept? 

We accept all major insurance carriers (including Medicare and Tricare) and participate with many local provider networks as well. We also see many major worker's comp carriers as well. Our staff will assist you in verifying physical therapy benefits through your insurance plan and make sure that any information regarding your insurance benefits is communicated proactively to you. 

Do you offer self pay options?

Competitive self pay options are also available. The rates for your initial evaluation and subsequent visits should be discussed with your physical therapist. Be aware that if you are covered under a health plan but decide to self pay, your payments will not reflect on your deductible and you will not be able to resubmit those services later to your insurance company. 

I have receive an Explanation of Benefits from insurance carrier, now what?

Understanding the Explanation of Benefits (EOB) can be difficult, but we are here to help! An EOB is a statement from the insurance carrier providing an overview of the cost for the services rendered. The EOB shows the amount billed to the insurance carrier and outstanding patient responsibility.
